Appointments

Appointments can be made by calling our office during normal business hours, which are 9 a.m. to 5 p.m.

After Hours & Emergency Care

If you are experiencing an medical emergency, dial 911 or go to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7-days a week. Non-emergency calls will be be answered during our normal business hours.

Prescription Renewals

You must have your pharmacist fax us the requistion prescription renewal. This can be faxed to 414-476-3242. Prescriptions will only be renewed or given during normal business hours. These can only be renewed through your pharmacy fax requistion. In some instances, we may require that the patient be seen in our office prior to medication renewal.

Financial

If you do not have medical insurance, services are payable at the time of your visit. We accept cash, checks, and most major credit and debit cards. You must bring proof of insurance and co-payments with you at the time of your visit. Please contact our billing office should you have any financial questions or concerns at 414-476-5493.
